Dog Tag License Application

Dogs are required to be licensed by the Town of Apple Valley. Dog license tags go on sale annually on January 1st and Expire December 31 of each year. Newly acquired dogs throughout the year must be licensed with 10 days of acquisition. Only (3) three dogs per household are permitted in the Town of Apple Valley. Temporary resident dogs with current license tags issued by another city or county do not need a Town of Apple Valley license. Owners of unlicensed dogs are subject to a citation and fine.

Fees per dog are $10.00 if spayed or neutered and $20.00 if not. License renewals are required by February 28th. A $5.00 late fee per month, per dog is charged after February 28th. (Maximum of $20.00 per dog.)

Tags may be purchased:

1. In person at the Town offices during regular business hours.
2. Online by filling in the webform below. After receipt of this form the Town will issue an invoice via email to you allowing you to mail in a check or pay online through Xpress Bill Pay.
3. By mail by returning the required documents to the Town of Apple Valley ATTN; Dog License 1777 N Meadowlark Dr., Apple Valley UT 84737
